 About Community Healthcaring Kitchener-Waterloo (The Health Centre)

 

The Health Centre provides a broad range of services aimed at improving the health of individuals and the wider community. Our services include medical and dental care, health promotion, illness prevention and outreach programming. Our expertise and resources are focused on working with those who experience health inequities and barriers to accessing health services. We work with some of the most complex clients in our region. Our highly skilled and diverse team provide services and supports that address social determinants of health, including poverty, inadequate housing, food insecurity, substance use, language, and culture, as well as stigma and social exclusion. Our team consists of Family Physicians, Nurse Practitioners, Registered Nurses, Social Workers, Dietitians, Chiropodists, Pharmacists, Psychiatrists, Health Promoters, Community Health Workers, Administrative Staff and Volunteers. The Health Centre is a member of the KW4 Ontario Health Team.

 

Position Summary

Utilizing specialized knowledge and skills, the NP provides patient-centred primary care to refugees, newcomers and patients from high priority neighbourhoods.  The NP will  assess, diagnose and prescribe treatment with an emphasis on health promotion and disease prevention.

 

Responsibilities

In collaboration with the team, the NP will:

  • Provide excellent evidence-based, and trauma-informed primary care to a culturally, and linguistically, distinct population in full scope of practice
  • Assess patients through booked appointments, walk-ins, and conduct new patient intakes
  • Develop policies and programs to further care at the centre
  • Support internal and external referrals to other health providers
  • Maintain accurate health records and up to date clinical notes in accordance with College of Nurses Standards and the Health Centre’s policies.
  • Enhance the learning and development of learners (students and international graduates) at the centre
  • Participate in the development, implementation, monitoring and evaluation of health promotion/disease prevention programs for individuals and client populations as determined by the Health Centre.

 

  • Work from a social determinants of health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being
  • Participate in research as determined by the Health Centre.
  • Adhere to all Health Centre policies.

 

Accountability

The position is accountable to the Manager, Primary Care

 

Qualifications

  • Knowledgeable about the refugee process, health equity and working with vulnerable populations preferred
  • Primary Care Nurse Practitioner in good standing with the College of Nurses of Ontario
  • Professional liability insurance
  • Current CPR certification
  • Proficiency with Electronic Medical Records (ideally, Telus PSSuites-PSS) and MS Office
  • Police Vulnerable Sector Check satisfactory to the Health Centre is required
